  • Proof of insurance, ID or citizenship is NOT required
  • Parent/guardian must come to appointment for people under 18 years
  • Appointments are optional (but may be scheduled at vaccinate.wi.gov)
  • Hmong interpreter available during clinic hours

Note: Reactions to the vaccine are very rare, yet out of abundance of caution we provide space for people to wait in their cars for 15 minutes and have medical personnel available in case someone has a reaction.
